Visa request cannot be processed without your original passport. The applicant's passport. The passport must:

Photocopy of Application Materials
A clear copy of the information page of the passport of the applicant.
Photograph
You must provide two colour, passport type photographs that meet the following criteria:
Be taken within the last three months and printed on high quality photo paper
Be taken against an all white background
Be 35 mm by 45mm in size
Provide the full frontal view of your head with your face centered in the middle and displaying a neutral (non-smiling) expression
You may wear no headwear except for religious purposes
Your photos should not be affixed to your application and they may not display evidence of adhesive or staples
Letter of Introduction
You must provide a Letter of Introduction from the applicant's Belgian employer. All letters must be printed on company letterhead that displays a Belgian postal address. Use the sample Letter of Introduction, which can be found here, to complete your letter.

Letter of Invitation
An invitation letter, written by the inviting company/sponsor in this destination. A copy is acceptable. The letter of invitation must:
Be on company letterhead paper
Include the full name of the applicant
Specify the purpose of journey
Specify the number of visits (entries) to the country of destination
Specify the length of the visa
Be stamped and signed
Proof of Travel Arrangements
The following documents must be provided:
A copy of your flight itinerary or your e-ticket
A hotel confirmation
Work history declaration
A declaration specifying your work experience. A sample is included in the CIBTvisas Application Pack. This declaration must:
Be fully completed
Be signed
Company Registration Details
A copy of the registration certificate of the company to be visited
Visa Validity
This is visa allows for a stay of up to 70 days.
Visa Application Form
One completed and signed application form is required. The form must:
Be completed in block capitals only
Be filled out in black or blue pen

Multiple Entry Applicants
